Have you been told that invasive surgery is your only option to fix your torn ACL? Getting your ACL replaced permanently alters the mechanics of the knee joint. You have other options when it comes to treating your ACL injury. In fact, about 70% of all ACL tears that currently get surgery could have been treated with a precise injection.
Your body is built to repair itself and our goal is to save your ACL, not replace it. We offer a non-surgical alternative for ACL tears that requires advanced injection skills using X-ray guidance, which may aide the healing of your ACL tear, while reducing pain and increasing function.
The procedure usually occurs over the course of one-day, although each patient and their ACL injury varies. There are some situations where a pre-injection will be used to define the status of the ligament architecture to better decide candidacy.
During the stem-cell procedure, a local anesthetic is applied, allowing the physician to draw bone marrow from the back of your iliac crest using precise imaging guidance. This is a specialized draw technique that is specific to Regenexx. This usually takes 30 to 40 minutes and is significantly less invasive than a bone marrow biopsy.
Once the marrow is drawn and processed by a Regenexx lab technician, you will have time to relax before the re-injection of the harvested stem cells into your ACL. The re-injection happens between three to six hours after the bone marrow aspirate is drawn. Specialized stem cell friendly local anesthesia is used before the re-injection takes place, using real-time imaging guidance which in this case is fluoroscopy. The MRI imaging is used to carefully map the torn ligament along with the x-ray contrast during the procedure. All of that provides a roadmap for the doctor to implant the stem cells into the damaged areas of the ligament. This part will usually take about 40 minutes to an hour. Patients may be asked to stay in the clinic 30 to 60 minutes after re-injection.
During the first 24 hours, you should limit your joint activity/range of motion and wear the recommended boot or knee brace.
Your joint will be sore for one to three days, but as most patients note, they see improvement to their ACL within the first month. Pain will become less severe and frequent and within five to seven days, patients can resume light activities and begin physical therapy.
Six to eight weeks later, patients can begin to resume more vigorous exercises and regular routines and will continue to see ACL improvement and healing month after month. We should see maximum ligament repair and tightening in younger patients at 3-6 months and in older patients up to 9-months.
However, results vary by patient and depend on the condition of the area treated, age, and general health of the patient.
